(a) Fort Apache Agency, Criminal Justice Unit, PO Box 560, Whiteriver, AZ 85941:  On or about 31 July 2013, and at times prior thereto, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2013:  A Criminal Investigator suffered spiral fracture of 4th metacarpal bone in left hand on 28 January 2013, cause unknown, which resulted in job restriction of an unknown number of days.    ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED

(a) Fort Apache Agency, Criminal Justice Unit, PO Box 560, Whiteriver, AZ 85941-0560:  On or about 31 July 2013, when requested by an authorized representative of the Secretary of Labor, the employer did not provide copies of the OSHA 300 Log for Calendar Years 2010, 2011, and 2012, and the current year to date within four business hours of request.
