29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: (a) Main Concourse - on or about August 12, 2013 - Ridgid Pipe Threader, model# 1224, serial# EB36599 1009; and Milwaukee Band Saw, model# 6230N, serial# C05AD09261565, in use with temporary wiring, were not protected by a Ground Fault Circuit Interrupter (GFCI).

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a) Main Concourse - on or about August 12, 2013 - Milwaukee Band Saw, model# 6230N, serial# C05AD09261565; and Ridgid Pipe Threader, model# 1224, serial# EB36599 1009, were not provided with a continual path to ground, in that the extension cord used to supply power had a broken ground pin.
