29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  KITCHEN AND LAUNDRY AREAS: On or about August 14, 2013, the employer failed to develop and implement an energy control program to include machine-specific procedures for machines with multiple energy sources, including electrical and mechanical; annual reviews of procedures by authorized employees; appropriate lockout hardware; and training for employees.  Equipment in the facility to be addressed in the lockout/tagout program includes, but is not limited to, washing machines and dryers in the laundry area and garbage disposals in the kitchen, all of which are powered through electrical disconnect boxes.
