29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.  a.  At the work site located at 4020 Business Park Drive, Columbus, Ohio, employees used powered industrial vehicles to pick and load customer orders for small restaurant wares, tabletop appliances and janitorial supplies.  The employer had not completed evaluations on the operators every three years as required by the standard.
