29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. a. On the top roof of the building there was a ladder set up for access to the Nate Auditorium roof which had a break in elevation of 28 1/2 inches thereby exposing the employees to a 9 foot 6 inch fall hazard.

29 CFR 1926.1053(b)(15): Ladders shall be inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use. a. On the Zanesville site where roofing work was being performed, the portable ladders were not inspected by a competent person for defects, damage to the ladder thereby exposing employees to a struck-by hazard.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired. a. On the south side of the building, the ladder used to gain access to the upper floor was missing a rope to raise and lower the ladder thereby exposing employees to a struck-by hazard. b. On the east side of the 2nd floor where employees accessed the roof, the aluminum ladder was missing the rope to raise and lower the upper part of the ladder thereby exposing employees to a struck-by hazard.
