Worksite located at 352 S. 291 Highway, Liberty, Missouri- Employees exposed to fall hazards were not protected from falling in that the equipment the employees were said to have used had not been inspected prior to use as evidenced by the employee's lanyard that had either activated or was damaged at some time prior to the inspection. Employees snaphook devices were so corroded with rust that the devices could not be opened without employees putting fingers in pinch point hazards in order to open the devices.
