29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zone during operating cycle:    a) In the Production Area, the crush point created by the closing of the dies was accessible through a 24 inch by 20 inch opening on the operator side of Injection Mold Machine #1.    b) In the Production Area, the crush point created by the closing of the dies was accessible through a 27 inch by 18.5 inch opening on the operator side of Injection Mold Machine #4.    c) In the Production Area, the crush point created by the closing of the dies was accessible through a 37.5 inch by 22 inch opening on the operator side of Injection Mold Machine #6.    d) In the Production Area, the crush point created by the closing of the dies was accessible through a 40 inch by 20 inch opening on the operator side of Injection Mold Machine #8.    e) In the Production Area, the crush point created by the closing of the dies was accessible through a 36 inch by 19 inch opening on the operator side of Injection Mold Machine #10.    f) In the Production Area, the crush point created by the closing of the dies was accessible through a 41 inch by 16.5 inch opening on the operator side of Injection Mold Machine #23.    g) In the Production Area, the crush point created by the closing of the dies was accessible through a 36 inch by 18.5 inch opening on the operator side of Injection Mold Machine #26.
