29 CFR 1926.950(c)(1): Employee(s) were permitted to approach and/or take any conductive object(s) without an approved insulating handle closer to exposed energized parts than shown in Table V-1:    On or about August 29th, 2013 employees were engaged in the tasks of erecting "guard structures" at the job site located 2520 yards north of the intersection of Currie Ln., and Northstar Rd. near Medicine Lodge, Kansas.  Employees setting an un-insulated pole were exposed to electrocution and electrical shock and burn hazards in that employees were not insulated from the pole and the pole was not insulated from the north phase of the 138,000 volt electrical power transmission lines.

29 CFR 1926.955(a)(5)(i): When setting, moving, or removing pole(s) using mechanized equipment near energized lines or equipment, precautions were not taken to avoid contact with energized lines or equipment:       On or about August 29th, 2013 employees were engaged in the tasks of erecting "guard structures" at the job site located 2520 yards north of the intersection of Currie Ln., and Northstar Rd. near Medicine Lodge, Kansas.  Employees setting an un-insulated pole were exposed to electrocution and electrical shock and burn hazards in that barriers and protective devices were not installed to prevent contact of the pole and the 138,000 volt electrical transmission line.

29 CFR 1926.955(a)(6)(ii): Lifting equipment shall be bonded to an effective ground or it shall be considered energized and barricaded when utilized near energized equipment or lines:     On or about August 29th, 2013 employees were engaged in the tasks of erecting "guard structures" at the job site located 2520 yards north of the intersection of Currie Ln., and Northstar Rd. near Medicine Lodge, Kansas.  Employees setting an un-insulated pole were exposed to electrocution and electrical shock and burn hazards in that the Nesco Prinoth GT 3000 with a Terex Commander 6060 digger derrick was not grounded.
