29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:     (a)    On August 29, 2013, at a job site located at N. 47th Troost, Kansas City, MO., employees were exposed to serious injuries, in that there was no ladder or other safe means of egress to and from the open excavation approx. 6 feet in depth.        Abatement certification for this violation is not required, abated during the inspection.

29 CFR 1926.651(e): Employees were not required to stand away from any vehicle being loaded or unloaded to avoid being struck by any spillage or falling materials:      On or about August 29, 2013, at jobsite 47th Troost Avenue in Kansas City, Missouri- Employee was performing work in a trench while a front end loader truck was also in the trench dumping rock and soil into the trench.       Abatement certification for this item is not required.

Troost, Kansas City, MO., employee was exposed to serious struck by and or engulfment injuries, in that there was no protective systems in use for work being done in the excavation.           Abatement certification for this violation is not required, abated during the inspection.
