29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:     (a)  Dohn Construction, Inc. at 1500 Tipton Drive, Windsor, CO 80550: On 9/3/13, the controlling employer did not ensure that fall protection systems were utilized by each employee engaged in fascia work at the roofs edge while working from a 5-in-12 pitch roof on a residential two story apartment. This condition exposed the employees to the hazard of falling up to 22 feet eave to ground.
