A Elevator shaft, second floor - On or about July 18, 2013, The opening for the elevator shaft had only a toprail being used as a guardrail for fall protection, presenting a fall hazard greater than 6 feet.. b) Construction site, Bldg. A second floor at common area - On or about July 18, 2013, The unprotected edge of the floor had a guardrail system tied in place with a piece of cloth, and with a mid-rail that was nearly touching the top-rail.
