29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables where run through holes in walls, ceilings, or floors: Warehouse: On or about September 13, 2013 and at times prior thereto, the flexible cord for the Datamax I Class label maker was run through the metal stud wall. This condition exposed the flexible cord to damage.

29 CFR 1917.111(b): Maximum safe load limits, in pounds per square foot (kilograms per square meter), of floors elevated above ground level, and pier structures over the water were not conspicuously posted in all cargo areas: Warehouse: On or about September 10, 2013 and at times prior thereto, where items were stored above the office area, load limits were not posted. This condition exposed the area to loading it may have not been designed for and incresed the potential for collapse.

29 CFR 1917.111(d): All walking and working surfaces in the terminal areas were not maintained in good repair: Warehouse: On or about September 10, 2013 and at times prior thereto, a 30 X 9 3/4 X 2 1/4 inch trough existed in the concrete floor inside the southwest overhead door. This condition exposed employee to trip and sprain hazards.
