29 CFR 1910.26(a)(3)(vii): The bottoms of the four rails are to be supplied with insulating non-slip material for the safety of the user.    On or about 9/11/2013 and before that time located in bay four of the shop area employees worked at heights in excess of four feet from the John Beam vehicle/rack scissors lift by ascending and  descending a five foot aluminum step ladder. The five foot aluminum step ladder was used with all four of the nonskid insulating boots damaged exposing the aluminum side rail leg material through the insulating boots.

29 CFR 1910.26(c)(2)(vii): Ladders having defects are to be marked and taken out of service until repaired by either maintenance department or the manufacturer.  On or about 9/11/2013 and before that time located in bay four of the shop area employees worked at heights in excess of four feet from the John Beam vehicle/rack scissors lift by ascending and  descending a five foot aluminum step ladder. The five foot aluminum step ladder was used with all four of the nonskid insulating boots damaged exposing the aluminum side rail leg material through the insulating boots without repair, replacement or being tagged out of service.

On or about 9/11/2013 and before that time located in bay four of the shop area employees worked at heights in excess of four feet from the platform of the John Beam vehicle rack scissors lift and then descended backwards without facing the five foot aluminum step ladder.
