29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  A. Precision Home Improvement worksite located at 4821 Whiteford, Sylvania, Ohio: On or about September 11, 2013, the employer did not ensure employees were protected from falls while installing weatherproofing on the east side of the two story residential structure.  The employees were exposed to a fall distance of approximatel 8.00 feet to the lowest level from the 4/12 pitch roof.

29 CFR 1926.1053 (b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.     A. Precision home Improvement worksite located at 4821 Whiteford, Sylvania, Ohio: On or about September 11, 2013, the employer did not ensure the side rails of the ladder was extended at least three feet above the landing surface located above the porch on residential structure.  Employees were exposed to falls while using the ladder to access the east side of the roof.
