29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: On or about September 19, 2013, a the Best Manufacturing facility in Jonesboro, Arkansas, employees who were operating powder coating equipment were wearing full face respirators. The employees had not been medically cleared to wear respirators. This condition exposed the employees to inhalation hazards.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about September 19, 2013, at the Best Manufacturing facility in Jonesboro, Arkansas, employees were fabricating custom sheet metal panels. Employees were operating machines in which the points of operation were not adequately guarded to prevent an employee from having any part of his body in the danger zone during operation. This condition exposed the employees to the hazard of amputations in the following instances: a) The Cincinnati shear b) The Chicago press brake

Monthly inspections, had been performed on the hook, but had not been documented. This condition exposed the employee to the the hazard of being struck by a falling load, in the event of a hook failure.
