29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    a)    On or about 9/30/2013, in the Southwest corner of the boiler room, storage on racks and below the fire extinguisher was not orderly, with insecure storage.  This area was used as a passageway for the boiler room.    b)    On or about 9/26/2013, in the Hog room, saw dust was covering floors and equipment including a floor cover that was open.  Additional wood scrap was on the floor outside of room by the barn cleaner.    c)    On or about 9/26/2013, saw dust and scrap were piled up in the aisle by black end hog. 29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    a)    On or about 9/26/2013, the Splicer No. 2 cut off area was not fully guarded to prevent access during operation, such as when clearing jams.  The approximately 24" guard door was not secured in place.    b)    On or about 9/26/2013, the Splicer No. 3 cut off area was not fully guarded to prevent access during operation, such as when clearing jams.    c)    On or about 9/26/2013, the Bunch clipper, 66"COE had an approximately 3" gap in the guard. The clipper knife was approximately 7" inside of that gap, (North side of machine).    d)    On or about 9/30/2013, the chipper in the saw mill was not guarded over the 23" x 19" opening. Pieces of wood were also being kicked back out of that opening.    ABATEMENT CERTIFICATION REQUIRED
