29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a. On or about October 17, 2013, at the above address job site, employees not wearing a safety harness with a lanyard attached to the boom or basket of an the aerial lift were exposed to a falll haxzard while gaining access to and from the roof edge of a building from the basket of the lift approximately 28 feet above a lower level.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:    a. On or about October 17, 2013, at the above address job site, employees performing steel erection work installing   metal roof panels on a walking/working surface approximately 28 to 33 feet above a lower level with   unprotected sides and edges were not protected from falling to lower level.    FLORIDA METAL BUILDING SERVICES, LLC WAS PREVIOSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.760(b)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER314019209, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS FINAL ORDER ON 01/26/2010, WITH RESPECT TO A WORKPLACE LOCATED AT 2500 SOUTH SR 27 IN CLERMONT, FLORIDA.
