The top and bottom ladder sections were separated and used individually to support a ladder-jack scaffolding system.

29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a) Reroofing project located at 2502 Blakeley Avenue in Eau Claire, WI; Fall protection systems were not utilized during reroofing operations being conducted on roofs sloped at 8/12. The house eaves were 9 feet, 4 inch and garage eaves were 8 feet above the lower level.    The Rock & Tait Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 98014, citation number 2, item number 1 and was affirmed as a final order on 12/27/2012, with respect to a workplace located at 2332 5th Street, Eau Claire, WI and OSHA inspection number 313177339, citation number 1, item number 1 and was affirmed as a final order on 04/19/2010, with respect to a workplace located at 2210 Gooder Avenue, Eau Claire, WI.
