29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)    On or about 9/20/2013, CMD-204 Winder was not guarded at the ingoing nip for the infeed rolls on the backside of the winder where a plexiglass guide was being adjusted.    b)    On or about 10/23/2013, CMD-204 Winder and CMD-202 Winder were being run without guarding at the location of where the push off device contacts the machine frame.    ABATEMENT DOCUMENTATION AND CERTIFICATION REQUIRED

29 CFR 1910.261(l)(9)(i): The nipping points of all drum winders and rewinders located on the operator's side were not guarded by either automatic or manually operated barrier guards of sufficient height to protect fully anyone working around them. No barrier guard was interlocked with the drive mechanism to prevent operating above jog speed without the guard in place. A zero speed switch was not installed to prevent the guard from being raised while the roll is turning.    a)    On or about 10/28/2013, PTP Rewinder had ingoing nip points for the bed rolls and Mt Hope roll that were approximately 12" from the frame of the machine and were not guarded.    ABATEMENT DOCUMENTATION AND CERTIFICATION REQUIRED
