29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Work Site Located at 2901 Cumings Street, Omaha, NE: Two pieces of equipment in the area were not equipped with machine guarding to prevent the employees from contacting rotating parts or other injurious movements. One of these pieces, Uniloy trimming machine, was guarded but the interlock for the device was easily bypassed allowing an employee to reach inside of the danger zone. a) Maintenance Department: MSC Milling Machine. b) Milk Bottle Manufacturing: Uniloy Machine Abatement certification is required for this violation. The guards have rusted to the point where there are holes which could allow an employee to reach into the danger zone causing a laceration or amputation. The holes in the guard act as a pinch point between the moving chain within and the guard itself. A) Case Dock Area B) Class Two Area Abatement certification is required for this violation. Two electrical boxes had openings of 3/4 inches on the side of the boxes which exposes the live parts within. The circuit in each box is energized to 220 volts according to labeling.
