29 CFR 1910.307(c) is applicable to employment by 29 CFR 1917.1(a)(2)(iv) that incorporates the General Industry requirement: Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location: (a) On or about November 14, 2013 the inspection revealed that employees were potentially exposed to serious injury from the use of a TPI Corporation heater that was not intrinsically safe or approved for a class II, division 2 location. The heater was located in the supercargo room, which was separated by a fire door from the structure housing the tail of the belt 3 barge inbound conveyor at Columbia Grain at 15660 N Lombard in Portland, Oregon. (b) On or about November 14, 2013 the inspection revealed that employees were potentially exposed to serious injury from two electrical outlets that were not intrinsically safe or approved for a class II, division 2 location. The outlets were located in the supercargo room, which was separated by a door from the structure housing the tail of the belt 3 barge inbound conveyor at Columbia Grain at 15660 N Lombard in Portland, Oregon.

29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water: (a) On or about November 14, 2013 the inspection revealed that the employer did not provide a swinging gate, offset, or chain at the passage through the railing on fixed ladderways. Located in several areas at the bin top catwalks, employees were potentially exposed to serious injury or death from falls of up to 28 feet to the lower level. (b) On or about November 14, 2013 the inspection revealed that the employer did not provide a guardrail at the head section 6B gate platform, potentially exposing employees to a fall of 7.5 feet to the lower level.

(a) On or about November 14, 2013 the inspection revealed that the in/out button labels on the new house trolley winch controls were not labeled. This crane was located on top of the new house bins at Columbia Grain on 15660 N Lombard in Portland, Oregon.

29 CFR 1917.45(k)(4): A record of monthly inspections was not maintained for 6 months in or on the crane or derrick, or at the terminal. (a) On or about October 30, 2013 the inspection revealed that the employer does not maintain monthly inspection records for the cranes and derricks at their terminal. There were five crane hoists for which monthly records were not maintained: three bin top hoists, one head house hoist, and one center tower hoist. All of the hoists were located at the facility at 15660 N Lombard in Portland, Oregon.
