29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Residence: On or about October 23, 2013, and at times prior, two workers without fall protection were exposed to falling from heights ranging from 8.9 feet to over 19.2 feet while installing roofing material. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1926.1053(b)(1): Where a portable ladder's length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device was not provided to assist employees in mounting and dismounting the ladder. (a) Ladder to first story roof: On or about October 23, 2013, and at times prior, two workers were exposed to falling 10 feet while using a ladder that did not extend above the first-story roof edge and was not secured at the upper landing level. (b) Ladder from first story to second story roof: Residence: On or about October 23, 2013, and at times prior, two workers were exposed to falling 19.2 feet while using a ladder that did not extend above the second-story roof edge and was not secured at the upper landing level. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).
