29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:      A)    American Fine Sinter Co., Ltd., located at 957 N. Maule Rd., Tiffin, OH 44883: On or about November 5, 2013, the employer failed to utilize machine-specific energy control procedures to control hazardous energy of the Tsukimura TK-40-08 mechanical power press (Model PT-40, Serial # 0199, 40 Ton). Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic, while performing activities such as, but not limited to, changing pads, setting-up and cleaning the press. Employees were exposed to amputation hazards.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Maule Rd., Tiffin, OH 44883: On or about November 5, 2013, the employer did not ensure that the Tsukimura TK-40-08 mechanical power press (Model PT-40, Serial # 0199, 40 Ton) was properly turned off/shut down while employees conducted activities such as, but not limited to, changing pads, setting-up and cleaning the press. Employees were exposed to injury from unexpected start-up and energy release.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.217(c)(3)(iii)(f): Mechanical power press(es) using presence sensing point of operation devices were not provided with guards to protect all areas of entry to the point of operation not protected by the presence sensing device(s):     A)    American Fine Sinter Co., Ltd., located at 957 N. Maule Rd., Tiffin, OH 44883: On or about November 5, 2013, the employer failed to ensure that all areas of entry into the point of operation of the Tsukimura TK-40-08 mechanical power press (Model PT-40, Serial # 0199, 40 Ton) were properly guarded. Employees were exposed to amputation hazards while entering the press point of operation through the door located on the back side.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Maule Rd., Tiffin, OH 44883: On or about November 5, 2013, the employer did not enforce the use of dies safety blocks while employees were changing and adjusting dies of the Tsukimura TK-40-08 mechanical power press (Model PT-40, Serial # 0199, 40 Ton). Employees were exposed to amputation hazards while changing and replacing dies of the press.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Maule Rd., Tiffin, OH 44883: On or about November 5, 2013, the employer did not ensure that employees were trained in the safe operation of Tsukimura TK-40-08 mechanical power press (Model PT-40, Serial # 0199, 40 Ton) located in the press room. Training was not provided regarding the hazards associated with control of mode selection. Operators were exposed to amputation hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
