29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. On or about 6 November 2013, and at times prior thereto, five employees within the fabrication area would use a table-mounted grinder on lifting bars in which the tongue guard for the abrasive wheel was more then 1/4th inch from the wheel surface, exposing the employees to the hazard of being struck by flying objects.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i. On or about 6 November 2013, and at times prior thereto, two employees within the crate building area utilized compressed air that was at approximately 60 psi at the nozzle to clean out excess sawdust buildup in and around a chop saw, exposing the employees to the hazard of being struck by flying debris or objects.

29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed. On or about 6 November 2013, and at times prior thereto, employees working within the fabrication area utilized an outlet receptacle box in which the energized conductors entering the box were not effectively closed with approximately 1.5 inches of wires visible, exposing the employees using the receptacle to an electrical hazard of approximately 120 volts.

On or about 6 November 2013, and at times prior thereto, employees within the weatherseal area used on a daily basis a pole-mounted fan that had exposed internally sheathed wires up to approximately 2 inches, exposing the employees to an electrical hazard of approximately 120 volts.
