29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) On or about 11/05/13 CSHO observed four men on the roof of a single family, two story home installing new roofing materials with no fall protection systems in place.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators. a) On or about 11/05/13 CSHO observed four men use a Tranzsporter, gas powered Platform Hoist as a ladder to access the roof when the employer did not ensure that materials hoist was used in a manner specified by its manufacturer.
