29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On or about 11/05/13 CSHO observed an employee, working as a ground man doing cleanup, working under coworkers installing new roofing materials when he was not wearing a hard hat.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:  a) On or about 11/05/13 CSHO observed four men on the roof of a single family, two story home installing new roofing materials with no fall protection systems in place.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators.     a) On or about 11/05/13 CSHO observed four men use a Tranzsporter, gas powered Platform Hoist as a ladder to access the roof when the employer did not ensure that materials hoist was used in a manner specified by its manufacturer.
