29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: On or about November 7, 2013 employees were exposed to hand and finger injuries while using the Baldor Grinder that had the work rest adjusted to approximately 1 inch from the wheel.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. On or about November 7, 2013, the guard on the Baldor grinder was adjusted to approximately 3/4 of an inch from the wheel, exposing employees to being struck by flying parts.

29 CFR 1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. On or about November 7, 2013, access to the electrical panel box labeled LA-3 in the machine shop was blocked, exposing employees to electrical hazards.
