29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.    On or about November 8, 2013, on the east side of the construction site, an employee engaged in metal framing activities on a commercial building worked from a man-lift without a body belt was exposed to a fall hazard of approximately 10 feet to the ground below.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used.     On or about November 8, 2013, on the east side of the construction site, employees engaged in metal framing activities utilized an extension cord with the outer sheathing split exposing internal wiring.  Employees were exposed to electrical hazards.
