On or about 10-31-2013, the employer failed to ensure that the bench grinder being used in the shop had a tongue guard that was properly adjusted to one fourth of an inch or less.

On or about 10-31-13, the employer failed to ensure that portable angle grinders that were being used in the shop were equiped with safety guards between the abrasive wheel and the operator.

Maintenance check includes an internal examination, except extinguishers with stored pressure.  On or about 10-31-13, the employer failed to ensure that the fire extinguishers on site had been inspected on an annual basis. The fire extinguishers had not been inpsected since 2010.
