Worksite located at 2300 Burlington Ave., North Kansas City, Missouri- Employee was observed working and walking near an unprotected side and edge without the use of any means of fall protection such as guardrail systems, safety net systems, or personal fall arrest systems.

Worksite located at 2300 Burlington Avenue, North Kansas City, Missouri- Employee was exposed to a fall hazard in that the portable ladder being used to access the roof awning level did not extend at least 3 feet above the upper landing surface.
