1917.43(c)(5): Powered industrial trucks shall be maintained in safe working order. Safety devices shall not be removed or made inoperative except as otherwise provided in this section. (a) Port of Detroit Terminal, Taylor Model TY620L, serial number 17646, a light beacon and an overhead rear view mirror had been removed. Employees were exposed to struck-by and crushing hazards. (b) Port of Detroit Terminal, Taylor Model TE-450M, serial number S-W4-26594, the mast tilt cylinders were leaking hydraulic fluid which affected operator visibility. Employees were exposed to struck-by and crushing hazards. (c) Port of Detroit Terminal, Taylor Machine Works forklift labeled 3605 was not maintained free of excess oil and grease on surfaces where employees may walk or stand for the purpose of egress or inspection, maintenance, and repair. Employees were exposed to slip and fall related hazards.
