29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):  a) North and west side of 424 Goepp Street - On or about November 21, 2013, employees were using an aluminium extension ladder as the base for a ladder jack scaffold that was located less than 1 foot from the energized entrance service drop.

29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  a) North and west side of 424 Goep Street- On or about November 21, 2013, employees were accessing the roof of a building using an extension ladder that did not extend at least three feet above the upper landing surface nor was it secured to prevent displacement.
