29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Center Bay: Employees were exposed to pinch points from the shear blade and hold downs as the AccurShear 12 foot shear was missing the rods that were part of the lower shear guard. The far right section of the shear was missing two metal rods that were part of the guard. On or about August 7, 2013 an employee broke two fingers when shearing a piece of metal with the lower portion of the guard absent. (b) ACX Pipe and Trim: Employees were exposed to the saw blade for the Makita 14 inch Chop Saw which did not have a lower guard to adjust to the material being cut.
