29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)   There was no frequent and regular inspection program being maintained in use by a competent person to ensure the jobsite, materials and equipment such as fall protection devices and systems were being maintained and utilized in a safe condition.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Located along various areas of  the steep pitched slope church roof deck in excess of 4/12 employees were observed performing associated roofing work tear off activities without conventional fall protection and were exposed to fall potentials in excess of 11.
