29 CFR 1910.335(b)(2): Barricades. Barricades used in conjunction with safety signs did not prevent or limit employee access to work areas exposing employees to uninsulated energized conductors or circuit parts. Conductive barricades may not be used where they might cause an electrical contact hazard: On or about December 4, 2013 employees were exposed to the hazards of electrical accident or electric shock when the wire technician/tester was working with live wire and testing control panels in the production shop when approximately twelve feet of space next to the wire technician/tester was unbarricaded and exposed employees to voltages from 120V to 480V.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards were not provided with, and did not use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed: On or about December 4, 2013 employee was exposed to the hazards of electric shock when not wearing gloves, safety glassses, nor proper clothing when working with live wire with voltages from approximately 24V through 480V and testing control panels in the production shop.
