On or about December 12, 2013, at the east side of the property, an employee was cutting trees using a Nifty SD-64 articulating boom lift without using a personal restraint system with a lanyard fastened to the basket, exposing himself to a 20 feet fall hazard.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     a.  On or about December 12, 2013, at the south side of the property, employees pushing fronds and branches through a Vermeer BC-1800 wood chipper were not wearing safety glasses, exposing themselves to eye injuries.     b.  On or about December 12, 2013, at the east side of the property, an employee using a Husqvarna chainsaw to cut branches from an aerial lift was not wearing safety glasses, exposing himself to eye injuries.
