29 CFR 1926.1101(h)(2)(i): The employer failed to implement a respiratory protection program in accordance with 29 CFR 1910.134(b) through (d) [except (d)(1)(iii)], and (f) through (m), which covers each employee required by 29 CFR 1926.1101 to use a respirator:  a) Community General Hospital, on or about 12/12/13. Requirements for 29 CFR 1910.134(k) were not implemented when an employee engaged in asbestos removal operations was not provided with annual training. The employee was last provided with training on 3/1/2011.   b) Community General Hospital, on or about 12/12/13: Requirements for 29 CFR 1910.134(m) were not implemented when an employee' records of respirator fit testing dud not include respirator size.  c) Community General Hospital, on or about 12/12/13: Requirements for 29 CFR 1910.134(d) were not implemented when the section on respirator selection did not include the employer's requirement to provide tight fitting powered air purifying respirators (PAPRs) upon employee's request and the fit testing and training requirements associated with the selection of PAPRs.  Abatement certification must be submitted for this item.
