(b) Jobsite: 10 West North Avenue, Pittsburgh, PA 15222, Near Front Entrance: On or before November 4, 2013, employees were exposed to electrical and/or fire hazards in that a single gang metal outlet box was not mounted. The receptacle was powering a quartz light.
