Employees were exposed to a struck-by hazard.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    1) Quality Inn Suites, 6597 Jonesboro Road, Morrow, GA - The employer did not train or certify the employee operating the Gradall model 524D-35 and serial 0277271 before allowing him to operate the equipment.  Employee was exposed to a struck-by hazard.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    1) Quality Inn Suites, 6597 Jonesboro Road, Morrow, GA - Employees were working inside a wooden platform on a Gradall forklift truck approximately 12 feet high conducting stucco work without fall protection. The Gradall forklift truck was being used to support a scaffold platform that was not designed or approved by the manufacturer.
