29 CFR 1910.134(i)(7): For oil-lubricated compressors, the employer shall use a high-temperature or carbon monoxide alarm, or both, to monitor carbon monoxide levels. If only high-temperature alarms are used, the air supply shall be monitored at intervals sufficient to prevent carbon monoxide in the breathing air from exceeding 10 ppm. The air supply for the abrasive blasting respirator used in the Blasting Area was not being monitored to assess levels of carbon monoxide in breathing air.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Particulates not otherwise regulated (PNOR) listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 15 mg/m3: An abrasive blasting employee working in the abrasive blasting booth on May 7, 2014 was exposed to particulates not otherwise regulated at an 8-hr time-weighted average level of 128.2 mg/m3, approximately 8.5 times the limit of 15 mg/m3; this limit established to prevent adverse health effects. The blaster used Black Magnum Coal Slag Abrasive media to blast metal parts for a customer. The exposure level is derived from three samples collected over a 76 minute period.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): An employee engaged in abrasive blasting was exposed to air contaminants in excess of the permissible exposure limit and feasible administrative and/or engineering controls were not implemented. General methods of control applicable in these circumstances include, but are not limited to, the following: a) Improvements in the ventilation system such as improved capture velocity. b) Addition of dust collection system with a bag house. c) An employee rotation program to reduce time of exposure. d) Substitution of abrasive blasting material.
