Division Street, Dover, DE - Employees were observed working on a 4/12 pitch roof of a 2-story building, approximately 20-25 feet above the ground, without fall protection. The employees were replacing shingles and working near the edge and side of the roof without any means of fall protection. The violation was observed on or about December 21, 2013.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards. The employees did not recognize the hazards of falling and each employee was not trained in the procedures to be followed in order to minimize these hazards: a) 630 W. Division Street, Dover, DE - Employees were not provided with training to recognize or minimize fall hazards while working in roofing. Employees were observed working on a 4/12 pitch roof of a 2-story building, approximately 20-25 feet above the ground, without fall protection. The employees were replacing shingles and working near the edge and side of the roof without any means of fall protection. The employees were not aware of alternative means of fall protection. The violation was observed on or about December 21, 2013.

Division Street, Dover, DE - Employees were observed accessing the roof using a Werner aluminum extension ladder that did not extend at least 3 feet above the upper landing surface. The employees used to ladder to replace roof shingles approximately 20-25 feet above the ground. This exposed the employees to fall hazards over 20 feet. The violation was observed on or about December 21, 2013.
