29 CFR 1910.303(b)(6): Deteriorating agents. Conductors and/or equipment were located in damp or wet locations; these components were not identified for use in damp or wet locations: A.) BFS Foods, Inc.; convenience store area - On or about 1/3/14, and times prior thereto, water from a leaking roof leaked onto the live electrical wiring. The electrical wiring included but was not limited to wiring for the Lottery machine, the Western Union machine and the coffee machines. B.) BFS Foods, Inc.; convenience store area - On or about 1/3/14, and times prior thereto, water from a leaking roof leaked onto electrical powered equipment. The electrical powered equipment included but was not limited to, the Lottery machine, the Western Union machine and the coffee machines.

29 CFR 1910.132(a): Protective equipment, including shields and barriers, were not used when necessary whenever hazards capable of causing injury and impairment were encountered: A.) BFS Foods, Inc.; convenience store area - On or about 1/3/14 and times prior thereto, employees were exposed to struck-by hazards from fall ceiling tiles. The roof leaked causing water to break the interior ceiling tiles. The ceiling tiles had fallen due to the weight of the water from the roof leaks. Devices such as shields, barriers, and/or restraints were not utilized to protect personnel from the falling ceiling tiles.
