29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: (a) Plant 80 - On or about November 15, 2014 the employer had developed furnace startup procedures for F-8007, (Procedure number-080-4401) which required employees to purge the firebox. The written purging instructions required a natural draft purge, with a negative draft pressure of at least 0.1, at the furnace arch for 15 minutes prior to lighting the burners. The purge cycle was not carried out prior to lighting the burners.
