29 CFR 1910.1000(a)(2):  Employee(s) were exposed to an airborne concentration of Carbon Monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 parts per million:    a)  Warehouse Building No. 1, on or about 12/5/13, an employee operating fork lift bringing bails of aluminum compressed cans into warehouse was exposed at a level of 333.47 parts per million (ppm) of carbon monoxide, over an eight hour time weighted average (TWA), approximately 6.67 times the OSHA permissible exposure limit of 50ppm. The sampling period was for 150 minutes. A zero increment is included for the 330 minutes not sampled. Employee was not wearing any type of respirator protection.
