29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) In the lobby on the second floor, on January 14, 2014, the employer did not ensure an employee working at the edge of a walking surgace that messured thirteen feet six inches from the ground level, while performing electrical work at a distance of approximately twelve inches from the edge was protected from falling, exposing the employee to the hazards associated with death, broken bones, sprains, lacerations, and bruises. 29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:   a) In the lobby by the entrance, on January 14, 2014, the employer did not ensure a hole in the concrete floor that measured twenty two inches wide by nineteen inches in length by five inches in depth was effectively covered to protect employees who were laying tile within six feet of the hazard from trips and falls, exposing the employees to the hazards associated with sprains, minor lacerations, and bruises.
