The path to ground from circuits, equipment, and enclosures was not  permanent, continuous, and effective.      a) On or about January 16, 2014, at the above located work place, employees worked with and around the following equipment which was not properly grounded:          a) A 120 volt Electrolux, Sanitaire Heavy Duty Commercial vacuum Cleaner Number 20A at room 2015.           b) A 120 volt Electrolux, Sanitaire Heavy Duty Commercial vacuum Cleaner Number 21A/19B at room 1951.           The employees using the equipment were thereby exposed to an electrical hazard.
