29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: a) Belmont Plating Works, Inc. - On January 22, 2014, a filter tank projected into the aisle at the cadmium/cyanide and nickel barrel plating lines next to the water rinse tank. The filter obstructed the aisle which is used as an exit route. No abatement certification or documentation is required for this item.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: (a) Belmont Plating Works, Inc. - Water was not available in an emergency at the eyewash located across from the Chromic Acid Rack Plating Line. The valve controlling the supply of water to the eyewash was turned off. Employees in this area were working at plating tanks containing hydrochloric acid, sulfuric acid and chromic acid which can cause burns, corneal damage and blindness. (b) Belmont Plating Works, Inc. - A shower was not available in an emergency across from the Chromic Acid Rack Plating Line. The handle used to activate the shower was tied up with a piece of wire. Employees in this area were working at plating tanks containing hydrochloric acid, sulfuric acid and chromic acid which can cause severe burns, sensitization and external ulcers. (c) Belmont Plating Works, Inc. - Water was not available in an emergency to both nozzles at the eyewash located near the cadmium and nickel barrel lines. Employees in this area were working at the plating tanks containing hydrochloric acid, sodium hydroxide and sodium cyanide which can cause severe burns, corneal burns, blindness and irritation. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation, Instance (c) (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
