(a) Shipping Department - On or about February 5, 2014 forklifts in need of servicing and repairs were not been removed from service.   Instance 1:  Yale #23 did not have an operable backup alarm  Instance 2:  Yale #24 did not have operable front headlights  Instance 3:  Hyster #9 did not have operable front headlights  Instance 4:  Hyster #10 did not have an operable backup alarm  Instance 5:  Hoist had an oil leak
