29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about February 6, 2014, employee operating an aerial lift at an industrial work site was wearing fall protection equipment but was not attached to the basket. This condition exposed employee to fall hazards of greater than 10 feet to ground level.
