29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i).    Jobsite: 302 Plantation Drive, Seaford, DE- Employee was buried inside a trench while making a residential connection to the main sewer line.  The employee was working inside a excavation approximately 40 feet long, 4 feet wide, and  the deepest part was approximately 6 feet deep with soil classified as type B. Observed on or about 2/7/2014.    Abatement Note:  Ensure protective system is provided such as sloping, benching, appropriate for soil type, or a trench box ior other type of protective system is used prior to authorizing an employee inside a trench 5 feet deep or greater or a trench under 5 feet deep with adverse condition and a potential for cave in.

The employee was working inside a excavation which was between 3 and 6 feet deep, approximately, without a ladder as means of egress.  Abatement note:  Ensusre to place a ladder or other means of egress at excavations that are 4 ft or more ind depth.

The employee was working inside a excavation approximately 40 feet long, 4 feet wide and between 3 and 6 feet deep. The spoil pile was between 0 and 5 inches from the edge of the excavation.  Abatement Note:  Ensure excavated material is placed no closer than 2 feet from the edge of the excavation.

The employee was working inside a excavation at 6 feet deep, approximately, without a hard hat. The spoil pile was within a foot from the edge of the excavation when it caved in.  In or about 2/7/14.  Abatement note:  Ensure employees wear a hard hat when working in areas where there is possible danger of head injury from impact.
