OSHA ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazard from unsupported members of a loading dock:      (a) The employer failed to ensure that the loading dock system, at 5920 West Bender Court in Milwaukee, Wisconsin, was working properly to prevent injuries from the uncontrolled fall of the lip section of the dock plate. The mechanical spring loaded system did not have a working safety system that restrict uncontrolled drop of the platform.  Employees were exposed to crushing injuries.
